POLICIES


We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. An insurance co-payment is required at check-in for each appointment. We accept cash, checks, MasterCard and Visa for your convenience. If we are a participating provider of your insurance company, we’ll bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.

Health Insurance: Klamath Orthopedic and Sports Medicine Clinic, and South Cascade Surgery Center, will partner with you in securing reimbursement from your insurance carrier(s) for your orthopedic care. We are participating providers with Medicare, Oregon and California Medicaids, Blue Shield/Blue Cross, Health Future, PHP, Lifewise, Pacific Source and others. Most insurance plans have specific guidelines. If necessary, contact your primary care physician for authorization or referral.  We will follow your insurance company’s guidelines and collect any co-pays at the time of service. Please be familiar with your responsibilities and your insurance company’s requirements.

If your treatment is due to a motor vehicle accident or worker’s compensation injury, please provide us with the name and address of the responsible insurance company and your claim number. It is essential that we have the referrals if required by your personal insurance carrier in the event your worker’s compensation claim is denied or your motor vehicle benefits are exhausted.

Billing Questions: If you have any questions pertaining to your bill, please contact our billing department during our normal office hours at (541) 884-3677. Our experienced billing personnel can assist you with complicated claims matters.

Prescription Refills: Prescription refills, including narcotics, are handled by the office only during regular business hours when our full records are available: (541) 884-3677. Renewals requested at other times will be filled only for extreme or emergency circumstances. Therefore, please plan ahead for medication needs you may have at night or over the weekend. Allow 24-48 business hours for refills of maintenance medications.

  • Courteous and respectful help from all members of our multi-disciplinary team;
  • Assurance that your personal information is confidential;
  • Information enabling you to make an informed decision about any surgery or procedure and to decide whether or not to consent to treatment before any treatment;
  • Complete, current, understandable information about your diagnosis and treatment, as well as possible alternatives, risks, and financial considerations of treatment;
  • Information explaining the possible consequences of refusing treatment;
  • Consideration without discrimination if you voice a grievance or suggest a change.
  • As a patient of South Cascade Surgery Center, you have the responsibility to:
  • Give accurate and complete information about illnesses, hospitalizations, past and current medicine use, allergies, and changes to your health;
  • Adhere to the pre-operative and post-operative guidelines provided by our staff and doctors;
  • Request information about anything that you do not understand;
  • Voice any concerns you may have so that we may address them immediately;
  • Inform South Cascade Surgery Center when you have a new name, address, phone number, insurance carrier or referring physician.
